How the night goes
You sit down once with the file and whatever you already have — the pill bottles, the wallet cards, the folder on the counter. You fill in what you know. You leave blanks where you do not.
The blanks are the point. They show you the four things to ask about at the next appointment, instead of the forty you were vaguely worried about.
Why paper
Because the person who needs it may not be you, and may not have your phone. A file in a drawer works when the battery is dead and the wifi is out and you are three states away.
